FAO Patients requesting Wegovy or Mounjaro weight loss injections
If you are requesting injections for the first time you will be required to upload a photo of your weighing scales with your weight clearly displayed and also a piece of paper in the frame with the date and your name clearly written on this. We also require you to upload a front or side on body image, please ensure this if fully clothed, it does not require your face to be visible in any way. These steps are required for verification of your weight.
If you are requesting Wegovy or Mounjaro weight loss injections but are currently already using the treatment could you please provide us with evidence of a photo of your current mounjaro or wegovy pen, with the label and the date on the label clearly visible for us to see. If you do not have the pen then could you please provide us with an image of your last order confirmation with the date clearly displayed of the last order.
Please could all patients also upload a proof of address or identity to your account too, this may be a letter or a form of ID such as driving licence or passport.
These photos can be uploaded to your account area once the order is completed where it will be securely stored.
Please ensure this is completed as soon as possible after your order to ensure there is no delay in processing your request.
